Sobre la terminología de la infección renal aguda en adultos: papel del médico radiólogo en su identificación y diagnóstico por imagen, con énfasis en TC

Introducción: las infecciones del tracto urinario (ITU) son infecciones bacterianas comunes, con fenotipos clínicos heterogéneos que varían desde infecciones benignas sin complicaciones hasta ITU complicadas y pielonefritis, e incluso urosepsis grave. La obtención de imágenes en las infecciones renales es compleja, dada la naturaleza de los hallazgos en la mayoría de los casos. Una evaluación cuidadosa de la situación clínica es esencial para seleccionar con precisión la técnica de imagen más informativa y por ende esa información debe ser lo más precisa posible.
Objetivo: ante la diversa nomenclatura existente y al acuerdo generado por la Sociedad de Urorradiología, en 1994, que recomendó una nomenclatura simplificada que se basa en el término tradicional y ampliamente comprendido de pielonefritis aguda (PNA), nuestro objetivo es enfatizar y reforzar esa nomenclatura con la evidencia actual.
Material y métodos: revisión prospectiva de dos años y tres meses con correlación clínico-tomo-patológica que incluyo pacientes con estudios de TC sin contraste (TCS), y postcontraste intravenoso (TCC) con el análisis de la terminología utilizada en la descripción de los hallazgos por TC y sus respectivos informes, con énfasis tanto de los hallazgos que se describen como de la impresión final.
Resultados: se definieron diferentes y diversas nomenclaturas, confusas algunas de ellas y sin un contexto claro. La PNA puede manifestarse como distensión perinefrítica asimétrica o leve agrandamiento renal en infecciones moderadas a graves. En la TC postcontraste intravenoso (TCC), la PNA puede observarse como áreas cuneiformes de baja atenuación con escasa diferenciación corticomedular, lo que indica hipoperfusión y edema. Las imágenes de fase tardía también pueden mostrar obliteración del seno renal, nefrogramas estriados y más. Se han observado complicaciones más graves de la PNA, como abscesos renales o perinefríticos.
Conclusiones: en nuestro universo de pacientes y sus estudios se identifico que debe reforzarse y establecerse un punto de acuerdo en el que en los respectivos informes, los hallazgos y la impresión final se ajusten a una terminología ya aceptada y reconocida. La TC es muy sensible para detectar PNA y definir su extensión, pero no es esencial para la PNA temprana sin complicaciones. El uso clínico de la TC puede ser apropiado en casos de fiebre persistente y leucocitosis que persisten más de siete días a pesar del tratamiento antibiótico.
