Efecto del consumo de isotretinoína sobre la espermatogénesis y la calidad seminal

Resumen

Introducción: el consumo de la isotretinoína, un derivado de la vitamina A, para tratar el acné severo, ha reportado efectos contradictorios sobre la espermatogénesis y la calidad seminal. Esta revisión de la literatura evalúa el impacto de la isotretinoína sobre la calidad seminal.

Relevancia y discusión: en humanos, la administración de isotretinoína en dosis terapéuticas ha demostrado consistentemente una mejoría en los parámetros seminales, incrementando la concentración, la movilidad y la viabilidad espermática. En contraste, los estudios en otros modelos animales, principalmente en ratas, muestran una toxicidad reproductiva. Dosis altas resultan en degeneración testicular, aumento de la apoptosis, reducción de las poblaciones de células de Sertoli y de Leydig. A pesar de que las concentraciones de isotretinoína detectadas en el semen humano son bajas, el conocido potencial teratogénico del fármaco remarca la importancia de continuar con precauciones estrictas para prevenir la exposición fetal.

Conclusión: en conclusión, la isotretinoína tiene efectos específicos sobre la calidad del semen, y aunque mejores resultados se observan en humanos usando dosis terapéuticas bajas, los datos obtenidos en modelos animales con dosis altas sugieren posibles efectos adversos en el sistema reproductivo.
