Impacto de la estenosis ureteroileal poscistectomía sobre la función renal: justificación para un seguimiento protocolizado

Resumen

Objetivo: evaluar el impacto de la estenosis ureteroileal (EUI) poscistectomía sobre la función renal y fundamentar la implementación de un seguimiento protocolizado.


Diseño: estudio retrospectivo en cohorte única de 111 pacientes sometidos a cistectomía radical con derivación urinaria (2002-2025). Se analizó la función renal mediante tasa de filtrado glomerular (TFG, ecuación CKD-EPI) en tres momentos: preoperatorio, diagnóstico de EUI y postratamiento. Variables incluidas: demográficas, técnicas quirúrgicas, manejo de EUI y complicaciones. Análisis estadístico con SPSS v26 (pruebas χ², t-Student, ANOVA de medidas repetidas, regresión logística).


Resultados: TFG disminuyó drásticamente desde preoperatorio (67.7±19.4 mL/min/1.73m²) a diagnóstico de estenosis (41.4±21.0; Δ=-26.3 mL/min; p<0.001), con recuperación parcial postratamiento (49.6±21.7; déficit residual: -18.1 mL/min vs. preoperatorio; p<0.001). Pacientes sin estenosis mantuvieron TFG significativamente mayor (74.9±17.6 vs. 49.6±21.7; p <0.001; d=1.31). Correlación entre diagnóstico tardío (>90 días) y mayor pérdida de TFG (ρ=0.487; p=0.003).

Limitaciones: diseño retrospectivo, heterogeneidad quirúrgica, muestra limitada para análisis multivariante.

Originalidad: primera cuantificación del daño renal agudo/residual tras EUI, identificación de ventana terapéutica crítica (<90 días) y justificación de seguimiento ecográfico protocolizado.

Conclusiones: la EUI causa deterioro renal irreversible. La alta incidencia (46 %) en cirugía laparoscópica y la ausencia de síntomas en 67 % de casos respaldan el cribado ecográfico temprano para prevenir fallo renal crónico.
