Efficacy of antibiotic prophylaxis in preventing post-cystoscopy urinary tract infection

Background: cystoscopy is one of the procedures frequently used in urogynecological consultation, the main disadvantage is the risk of a urinary tract infection (UTI). This research aimed to determine the effectiveness of the use of the prophylactic antibiotic in the prevention of UTIs in patients undergoing cystoscopy.
Methods: a prospective, longitudinal, comparative and analytical quasi-experimental study was carried out at the Hospital Civil de Guadalajara Fray Antonio Alcalde, in Guadalajara, Mexico. Patients over 18 years of age who underwent cystoscopy were included and assigned to 3 groups (Group 1: Ciprofloxacin 500 mg PO DU, Group 2: Nitrofurantoin 100 mg PO DU and Group 3: No treatment). A week later, the patients were evaluated with a urine culture.
Results: the study included 186 patients undergoing cystoscopy, of whom 63 were administered ciprofloxacin, 62 nitrofurantoin, and 61 were not treated. The average age was 56 years. A significant difference (p = .013) was observed in the reduction of UTIs with nitrofurantoin (RR = .13, 95 % CI .02 to .97) compared to group 2. A difference (p = 0.13) was also observed in the percentage with positive urine culture among patients in the nitrofurantoin 7 group (12.3 %) and no treatment 19 (33 %); (RR = .37, 95 % CI: .17 to .81).
Conclusions: the study confirms a significant difference in the prevention of UTIs with the use of nitrofurantoin, which is why this drug is considered useful in post-cystoscopy antimicrobial prophylaxis in the population of the west of the country.
